Job summary

Journey is hiring a dedicated MDS Assistant in Saint Matthews, KY for a full-time, on-site role. The position supports the Resident Assessment Process, CAAs, and the creation of individualized care plans to meet federal and state requirements.

The ideal candidate is a licensed RN with knowledge of MDS 3.0, happy to collaborate with our Care Team to deliver high-quality outcomes for residents and families. Journey offers competitive benefits and a supportive work environment.

Qualifications

• Active Registered Nurse license.
• Experience with MDS 3.0 and CAAs preferred.
• Knowledge of resident assessment and care planning processes.

Responsibilities

• Participates in the Resident Assessment Process and CAAs.
• Contributes to developing individualized resident Care Plans and outcomes as required by regulations.

MDS Assistant participates in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs). Participates in developing individualized resident Care Plans, identifying the needs of the residents and projected outcomes as required by Federal and State regulations.

Required Qualifications

• A professionally trained Registered Nurse that has earned a certificate/diploma or degree in nursing.
• Must hold a current nursing license in the state of employment.
• Working knowledge of the MDS 3.0 (current version of minimum data set).
