Base pay range

$75,000.00/yr - $80,000.00/yr

Job Summary

The MDS Assistant is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents to ensure appropriate reimbursement. They will coordinate nursing, dietary, social, recreational, restorative, rehabilitation, and physician services to ensure proper assessment and reimbursement. Additionally, they will develop, implement, monitor, and supervise the restorative nursing program, acting as a liaison between therapy and nursing, and promoting a restorative nursing philosophy across departments.

Essential Duties and Responsibilities
  1. Assess and determine the health status and care needs of all new admissions.
  2. Ensure accurate and timely completion of all MDS assessments, including PPS Medicare, quarterly, annual, and significant change assessments.
  3. Communicate the level of care for new residents to all disciplines.
  4. Coordinate interdisciplinary participation in completing the MDS for each new admission within regulatory timeframes, ensuring documentation is complete and compliant.
  5. Maintain an accurate schedule of MDS assessments with proper reference dates.
  6. Ensure accurate data entry and electronic submission of MDS assessments.
  7. Assist in developing and implementing nursing care policies, procedures, and standards in line with the facility's goals.
  8. Collaborate with the Director of Nursing to assess and improve nursing care quality.
  9. Perform daily rounds to evaluate patient care, monitor staff, and address nursing needs.
  10. Stay informed of legal and regulatory aspects of nursing practice, incorporating this knowledge into supervision and patient care evaluation.
  11. Conduct rehabilitation assessments for new admissions and ongoing evaluations quarterly or as needed.
  12. Oversee functional evaluations and collaborate with nurses and therapists to develop restorative care plans.
  13. Ensure restorative measures are reflected in care plans and carried out effectively.
  14. Work closely with therapy departments to implement discharge recommendations.
  15. Review weekly documentation for residents receiving restorative services, ensuring proper ADL documentation.
  16. Oversee incontinent management programs.
  17. Participate in care planning and interdisciplinary meetings, providing feedback on restorative nursing progress.
  18. Coordinate staff orientation and ongoing education on restorative nursing principles.
  19. Participate in programs aimed at reducing restraints and fall risks.
  20. Set up documentation flow sheets for restorative nursing measures.
  21. Supervise restorative nursing staff and act as a liaison with therapy providers.
  22. Participate in patient care planning, CMI meetings, and serve as Medicare Services Coordinator (MDS Coordinator) as needed.
  23. Ensure high-quality, compassionate care that respects patient rights and needs.
  24. Perform other duties as assigned.
Qualifications
  • Registered Nurse with current, unencumbered state licensure.
  • Preferred experience in Long Term Care.
  • Proficient in English (reading, writing, speaking, understanding).
Physical Demands
  • Ability to sit, stand, bend, walk, and lift/move up to 25 pounds.
  • Sufficient visual and auditory capacity for communication.
  • Moderate noise level in the work environment.
