Job summary

Ditmas Park Nursing & Rehab in Brooklyn, NY is seeking an experienced MDS Assessor to join our clinical team. The ideal candidate will perform accurate MDS assessments, support individualized care planning, and ensure compliance with federal and New York State regulations.

The MDS Assessor will coordinate with nursing, therapy, social services, dietary, and other departments; participate in care-plan meetings; and ensure PDPM-based reimbursement is properly documented.

Qualifications

• Active New York State RN license.
• Previous MDS experience in a skilled nursing or long-term-care facility.
• Strong understanding of MDS 3.0, PDPM, and care planning.
• Excellent organizational and documentation skills.
• Ability to meet deadlines and work collaboratively with an interdisciplinary team.

Responsibilities

Location: Ditmas Park, Brooklyn, NY

Schedule: Monday–Friday, 9:00 AM–5:00 PM

Salary: $120,000–$130,000 per year

Ditmas Park Nursing & Rehab is seeking an experienced and detail-oriented MDS Assessor to join our clinical team.

The MDS Assessor will be responsible for completing accurate and timely resident assessments, supporting individualized care planning, and ensuring compliance with federal and state regulations.

Responsibilities:

• Complete MDS assessments accurately and within required timeframes

• Review resident documentation and clinical records

• Coordinate with nursing, therapy, social services, dietary, and other departments

• Participate in interdisciplinary care-plan meetings

• Ensure assessments reflect each resident's condition and level of care

• Monitor documentation for accuracy, consistency, and compliance

• Support proper reimbursement under the Patient-Driven Payment Model (PDPM)

• Maintain compliance with CMS and New York State requirements

Requirements:
