Job summary

Triboro Center is hiring an MDS Assessor to work in-person at its Skilled Nursing Facility in Bronx, NY. The role requires a valid New York RN license and prior MDS/care planning experience.

Responsibilities include completing MDS assessments, initiating care plans, and maintaining accurate documentation while ensuring confidentiality and teamwork within a 405-bed rehabilitation and skilled facility.

Qualifications

• Must hold a valid New York RN license.
• MDS and care planning experience preferred.
• Long-term care experience required.
• Must be highly organized, professional and motivated.
• Solid computer skills.
• Excellent communication and teamwork abilities.

Responsibilities

• Complete accurate MDS assessments and care plans as assigned.
• Initiate and support care planning activities.
• Create and distribute monthly care plan calendars promptly.
• Maintain and update all care plans and assessments as required.
• Monitor and audit clinical records for accuracy and timeliness.
• Inform DON of persistent issues related to non-compliant documentation.
• Protect confidentiality of resident and facility information at all times.

Required: Registered Nurse (RN) License

DUTIES:

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times

REQUIREMENTS:

• MDS Experience Preferred
• Valid New York RN License
• Long Term Care Experience Required!
• Must be highly organized, professional & motivated
• Should have solid computer skills
• Excellent communication skills
• Should be friendly and a team worker
