Qualifications

• Active New York State RN license is required.
• Previous MDS experience in a skilled nursing or long-term-care facility is required.
• Strong understanding of MDS 3.0, PDPM, and care planning.
• Excellent organizational and documentation skills.
• Ability to meet deadlines and work collaboratively with an interdisciplinary team.

Responsibilities

• Complete MDS assessments accurately and within required timeframes.
• Review resident documentation and clinical records.
• Coordinate with nursing, therapy, social services, dietary, and other departments.
• Participate in interdisciplinary care-plan meetings.
• Ensure assessments reflect each resident’s condition and level of care.
• Monitor documentation for accuracy, consistency, and compliance.
• Support proper reimbursement under the PDPM.
• Maintain compliance with CMS and New York State requirements.
