Triboro Center in Bronx, NY is hiring an MDS Assessor. The role requires a valid New York RN license and experience with MDS and long-term care. You will complete assessments, devise care plans, and monitor clinical records while maintaining confidentiality.

The candidate should be organized, professional, and a strong team player with excellent communication skills to support residents and staff in a busy rehab and skilled nursing facility.

Responsabilidades

• Complete accurate MDS assessments and care plans as assigned.
• Initiate care plans and supporting activities.
• Create and distribute monthly care plan calendars in a timely fashion.
• Maintain and update all care plans and assessments as required.
• Monitor and audit clinical records for accuracy and timeliness.
• Inform DON of persistent issues related to non-compliant documentation.
• Protect confidentiality of resident and facility information at all times.

Triboro Center is hiring an MDS Assessor in Bronx, NY.

Required: Registered Nurse (RN) License

DUTIES:

• Completing accurate assessments, MDS & care plans as assigned

• Initiating care plans and supporting activities as assigned

• Creating and distributing monthly care plan calendars in a timely fashion

• Maintaining & updating all care plans and assessments as required

• Monitoring & auditing clinical records, ensuring accuracy & timeliness

• Informing DON of persistent issues related to non-compliant documentation

• Protecting the confidentiality of Resident & Facility information at all times

REQUIREMENTS:

• MDS Experience Preferred

• Valid New York RN License

• Long Term Care Experience Required!

• Must be highly organized, professional & motivated

• Should have solid computer skills

• Excellent communication skills

• Should be friendly and a team worker
