Job summary

Park Nursing Home in Rockaway Park, NY, is hiring an MDS Assessor to work in-person at its 196-bed Skilled Nursing Facility by the boardwalk, with easy access to subway, bus and ferry. The position requires a valid New York RN license and long-term care experience; MDS experience is preferred.

Responsibilities

• Perform accurate MDS assessments and care plans as assigned.
• Initiate and support care plans and related activities.
• Create and distribute monthly care plan calendars in a timely manner.
• Maintain and update all care plans and assessments as required.
• Monitor and audit clinical records for accuracy and timeliness.
• Inform Director of Nursing of any non-compliant documentation issues.
• Protect the confidentiality of resident and facility information at all times.

Park Nursing Home is hiring an MDS Assessor to work in-person at its Skilled Nursing Facility in Rockaway Park, NY.

We are a 196 bed facility located right by the boardwalk at Rockaway Beach. We have a beautiful view of the Atlantic Ocean and we are accessible to the subway, water ferry and buses.

Required: Registered Nurse (RN) License

DUTIES:

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times

REQUIREMENTS:

• MDS Experience Preferred
• Valid New York RN License
• Long Term Care Experience Required!
• Must be highly organized, professional & motivated
• Should have solid computer skills
• Excellent communication skills
• Should be friendly and a team worker
