Job summary

Boro Park Center in Brooklyn, NY is hiring an MDS Assessor to join our team. The position requires MDS 3.0 experience and a valid New York RN license, with long-term care background highly valued.

You will perform accurate assessments, maintain care plans, monitor records, and ensure timely documentation while protecting resident confidentiality. We offer a supportive environment and opportunities to grow within our Centers Health Care network. Daily Pay is available.

Responsibilities

• Completing accurate assessments
• Supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining and updating all care plans and assessments as required
• Monitoring and auditing clinical records, ensuring accuracy and timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident and Facility information at all times
