Job summary

Highbridge Healthcare is seeking a skilled Registered Nurse with MDS experience to conduct comprehensive assessments and contribute to care planning in a long-term care setting.

You will lead interdisciplinary team discussions and ensure accurate MDS documentation for regulatory compliance and proper reimbursement.

Qualifications

  • RN license required.
  • Experience in MDS role or long-term care.
  • Strong communication, organizational, problem-solving, and leadership abilities.
  • Proficient in MDS 3.0 and computer literacy.
  • Knowledge of federal/state regulations and the RAI process.

Responsibilities

  • Assess residents' physical, psychological, and functional status on admission, periodically, and discharge.
  • Develop, implement, and monitor individualized care plans with short and long-term goals.
  • Complete and submit mandated MDS forms and required reports accurately.
  • Lead or participate in interdisciplinary team meetings to discuss resident needs.
  • Ensure data accuracy for Medicare/Medicaid payments and RUG considerations.

Job description

  • Assessment:Conduct comprehensive assessments of residents' physical, psychological, and functional status upon admission, periodically, and at discharge.
  • Care Planning:Develop, implement, and monitor individualized care plans with short and long-term goals for residents.
  • Documentation:Accurately complete and submit mandated MDS forms and other required reports.
  • Team Collaboration:Lead or participate in interdisciplinary team meetings (nursing, PT, dietary) to discuss resident needs and care strategies.
  • Reimbursement:Ensure data accuracy for Medicare/Medicaid payments, influencing Resource Utilization Groups (RUGs).
Key Qualifications
  • License: Registered Nurse (RN)
  • Experience:Proven experience in an MDS role or long-term care setting.
  • Skills:Strong communication, organizational, problem-solving, and leadership abilities, plus computer literacy (MDS 3.0 proficiency is key).
  • Knowledge:Deep understanding of federal/state regulations and the Resident Assessment Instrument (RAI) process.
