Job summary

Hale Makua Health Services on Maui is seeking a compassionate Social Services Assistant to support our kūpuna within a long-term care facility. You will assist with resident advocacy, care coordination, and psychosocial support to ensure person-centered, culturally sensitive care.

You will collaborate with Social Workers, nursing staff, residents, and families to promote dignity, safety, and quality of life throughout admission, stay, and discharge. Join our nonprofit team on Maui.

Qualifications

• Bachelor's degree or equivalent in human services field or related area.
• One year of experience in a health care setting working directly with individuals.
• Current and valid 2 Step TB clearance and health review.

Responsibilities

• Completes resident social histories, discharge plans, and develops care plans as part of the interdisciplinary team.
• Provides ongoing psychosocial assessment and assists residents with access to additional services during their stay.
