Job summary

Qualifications

Responsibilities

• Create and implement a written plan of care addressing patient needs and goals.
• Modify plan based on reassessment and patient progress.
• Delegate tasks to team members within scope of practice.
• Observe and document interventions; communicate plan through EMR and verbal reports.
• Provide patient care including IV starts, blood administration, and point-of-care testing per CLIA.
• Administer medications per physician orders and monitor for adverse effects.
• Transition patients with updated care plans; provide timely reports to incoming staff.
• Inform physicians and care team of changes in condition or abnormal findings.
• Provide discharge education to patient and family; may precept new staff.
