Major Hospital is seeking a Maternity Care Case Manager/Lactation Consultant. The role leads the nursing team, coordinates maternal child patient care, and collaborates with the healthcare team to optimize outcomes. Requires RN license and nursing background with lactation experience preferred.

The position involves day shift coverage with a mix of direct patient care and office duties, reporting to the Director of Maternal ChildServices. Experience in lactation and OB is preferred.

Qualifications

• Experience gained through an approved School of Nursing plus an appropriate period of hospital orientation to the assigned patient care area and to personnel.
• Lactation counseling experience preferred.
• OB experience preferred.

Responsibilities

• Lead the healthcare team and coordinate nursing care for maternal child patients.
• Plan, direct, and coordinate nursing care using the nursing process.
• Collaborate with health care professionals to facilitate patient movement across the continuum.
• Coordinate pre-admission, discharge, and follow-up care coordination.
• Provide lactation management services to patients and families in collaboration with the team.
