Job summary

Maternity Care Coalition is seeking a Case Manager to provide culturally sensitive one-on-one health and parental education for families affected by NAS and substance exposure. This role involves hospital, home, and community-based case management across several Pennsylvania counties, with travel as needed.

The position requires a relevant degree and at least two years of experience in substance use and maternal/child health, along with strong documentation skills and the ability to coordinate

Qualifications

• Two-plus years of experience working with substance abuse and community health issues.
• Associate’s degree required; Bachelor’s degree preferred.

Responsibilities

• Collaborate with obstetrics and neonatal divisions of regional hospitals to support women with opioid dependency and NAS infants.
• Provide intensive case management, including assessment, referrals, follow-up, and monitoring.
• Conduct home visits, office visits, and community outreach with regular phone/text contact.
• Ensure timely scheduling of postpartum and well-child appointments and accompany clients to appointments.
