Job summary

NYU Grossman School of Medicine in New York invites applications for a Community Health Rep to support a maternal health initiative bridging education, outreach, and care coordination across NYC communities.

You will collaborate with Community Health Workers, clinical teams, and community partners; assist with protocol development, recruitment, and training material refinement; and help maintain partnerships while delivering health coaching and case management within the study framework.

Qualifications

• High School Diploma and 1 year of related office experience.
• Effective oral, written communication and interpersonal skills.
• Haitian Creole language preferred.

Responsibilities

• Participate in trainings including nutrition, stress management, goal setting, weight management and outreach.
• Translate and develop tailored flyers and materials.
• Liaise between pregnant participants, healthcare providers, and the NYU research team.
• Conduct group and 1:1 sessions on maternal health with pregnant participants.
• Maintain a caseload of ~25 participants and coordinate appointments.
• Document steps in study logs and update recruitment data.
• Provide individual counseling and support; assist with goal setting.
• Assess social needs and connect clients with community resources.
• Develop written action plans describing identified needs and interventions.
• Enter client data into the web-based platform regularly.
• Conduct interviews and surveys with study participants.
• Complete baseline assessments and follow-up calls.
• Keep participants motivated and engaged; adhere to safety protocols.
