Job summary

MECA Therapies LLC in Rio Arriba, NM seeks a Delegated Care Coordinator for its Maternal Health Program to coordinate care for pregnant and postpartum individuals, connect families with resources, and support access to prenatal and postpartum services.

You will work with healthcare partners, travel within service areas, perform home and virtual visits, document thoroughly, and uphold HIPAA in a mission-driven role improving maternal and infant health across New Mexico.

Qualifications

  • Bachelor's degree or equivalent experience in a related field.
  • Minimum of two years of experience in case management, care coordination, social services, healthcare, or related area.
  • Valid driver's license, reliable transportation, and ability to travel within assigned service areas.
  • Strong organizational, communication, and documentation skills.
  • Ability to maintain confidentiality and comply with HIPAA requirements.

Responsibilities

  • Conduct comprehensive intake assessments to identify medical, behavioral, social, and environmental needs.
  • Develop individualized care plans to support healthy pregnancies and positive birth outcomes.
  • Coordinate care with physicians, hospitals, and community agencies.
  • Perform home and virtual visits and follow-ups to monitor progress.
  • Educate members on prenatal care, postpartum recovery, infant care, breastfeeding, nutrition, and maternal wellness.
  • Connect families with transportation, housing, food programs, childcare resources, and behavioral health services.
