Job summary

NaphCare, Inc. is seeking a MAT Nurse Practitioner to join the team at Mesa County Jail in Grand Junction, CO. The NP will deliver MAT services for opioids in a correctional setting, focusing on buprenorphine/naloxone and comprehensive patient care.

The role requires current CO NP licensure, DEA registration, and a collaborative agreement with the on-site physician. You will work with correctional staff and healthcare professionals to ensure safe, compliant, and effective treatment plans.

Qualifications

• Advanced nursing education (Master's or Doctorate).
• MAT/OUD treatment experience required.
• Current licensure as a Nurse Practitioner.
• Active Colorado NP license and DEA registration.
• Collaborative agreement with on-site Physician.
• BLS CPR Certification (AHA).
• Board certification recognized by ABMS.
• CARN-AP preferred.

Responsibilities

• Provide comprehensive MAT services for incarcerated individuals with OUD, focusing on buprenorphine/naloxone.
• Conduct initial and ongoing clinical assessments for OUD and readiness for treatment.
• Initiate, monitor, and adjust Suboxone therapy per guidelines and policies.
• Manage MAT induction, stabilization, maintenance, and tapering phases.
• Collaborate with correctional healthcare, custody, mental health, and counselors for coordinated care.
• Monitor adherence, response, and diversion risks in a correctional setting.
• Educate patients on OUD, MAT, relapse prevention, overdose risk, and continuity of care.
• Participate in discharge planning and referrals to community MAT providers.
• Maintain compliant documentation in EHR and MAT reporting.
