Job summary

Elderberry Health, an integrated primary care clinic in Tennessee, seeks a Marriage and Family Therapist, Professional/Mental Health Counselor, or Social Worker to join a growing behavioral health team.

New grads welcome; you will conduct individual sessions, develop treatment plans, perform assessments, document care, and provide crisis intervention under supervision to support patient safety and well-being.

Qualifications

• Master's degree in Social Work or Counseling is required.
• Must hold a valid associate license in Tennessee (LMFT-A, LPC-MHSP temp, LMSW).

Responsibilities

• Conduct and document individual sessions under supervision.
• Perform initial and follow-up patient assessments.
• Prepare individualized treatment plans.
• Complete patient documentation in a timely and accurate manner.
• Provide crisis intervention and case management as needed.
• Refer patients to other programs or facilities as necessary.
