Job summary

MUSC Health in Charleston, SC, is recruiting a Medical Social Worker (MSW) to coordinate discharge planning and collaborate with the health care team to achieve safe transitions and improved patient outcomes.

The MSW conducts assessments of patients and caregivers during hospitalization and outpatient follow-up, identifies risks after discharge, and applies interventions to reduce those risks while connecting families to outpatient providers.

Qualifications

• Master's degree in social work or social welfare program accredited by the CSWE.
• MSW licensure by the South Carolina Board of Social Work Examiners.
• Certification in case management preferred.

Responsibilities

• Coordinate discharge process with the health care team to ensure a safe transition and targeted patient outcomes.
• Complete patient and caregiver assessments throughout hospitalization and follow-up.
• Identify post-discharge risk factors and apply interventions to lower risk; bridge discharge to outpatient providers.

Education
