Job summary

One Senior Care in Meadville, PA is seeking a Bachelor Social Worker to support participants and caregivers through care coordination, community resources, and social service needs, working closely with the Social Work team and IDT.

This role emphasizes relationship-building, organization, and compassionate communication in a team-centered environment focused on person-centered care. Training and mentorship are provided to grow professionally.

Qualifications

  • Bachelor’s Degree in Social Work required.
  • Experience with frail or elderly population is a plus.
  • Healthcare, case management, or social services experience preferred.
  • Interdisciplinary team collaboration experience is a plus.

Responsibilities

  • Support IDT with care planning and participant service coordination.
  • Assist with participant assessments and care plan updates under supervision.
  • Maintain accurate documentation and participant files.
  • Coordinate referrals and community resources for participants.
  • Provide supportive communication and address psychosocial concerns.
  • Schedule care plan meetings, reassessments, and IDT meetings.
  • Prepare meeting materials and record minutes.
  • Support care plan tracking and follow-up activities.
  • Assist with participant check-in and check-out as needed.

Job Summary:

As a Bachelor Social Worker, you’ll bring compassion, organization, and strong communication skills to every interaction. You’ll support Participants and caregivers by assisting with care coordination, community resources, documentation, and social service needs while working closely with the Social Work team and Interdisciplinary Team (IDT).

This role is ideal for someone who enjoys building relationships, staying organized, and helping seniors navigate services and support systems. You’ll be part of a team-centered environment where collaboration, respect, and person-centred care come first.

Schedule:

First shift, full-time hours. Specific work hours will be established in consultation with your supervisor.

Duties and Responsibilities:
  • Support the Interdisciplinary Team with care planning and Participant service coordination.
  • Assist with gathering information for participant assessments and care plan updates under supervision of the Social Work team.
  • Maintain accurate documentation, records, and participant files in compliance with organizational standards.
  • Coordinate referrals, community resources, and supportive services for Participants and caregivers.
  • Provide supportive communication and assist with addressing psychosocial concerns.
  • Schedule care plan meetings, reassessments, appointments, and IDT meetings as assigned.
  • Prepare meeting materials, maintain documentation, and record meeting minutes.
  • Support care plan tracking and follow-up activities.
  • Assist with Participant check-in and check-out processes as needed.
  • Communicate Participant updates and concerns to the appropriate team members.
  • Build positive relationships with Participants, caregivers, staff, and community partners.
  • Participate in quality improvement activities, ongoing education, and team training initiatives.

Education and Experience:
  • Bachelor’s Degree in Social Work required.
  • Experience working with the frail or elderly population is a plus — but not required.
  • Previous experience in healthcare, case management, or social services support preferred.
  • Experience working in an interdisciplinary healthcare environment is a plus.
Physical Requirements:
  • Must be able to move intermittently throughout the workday.
    Must be able to sit, stand, bend, lift, and move throughout working hours.
    Must be able to manage frequent interruptions and changing priorities.
    Must remain alert to Participant safety needs at all times.
    May be required to assist with Participant mobility or evacuation needs as necessary.
