PURPOSE AND SCOPE:
Provides psychosocial services to patients treated by the facility including in-center and home dialysis patients (if applicable) utilizing Social Work Theory of Human Behavior and accepted methods of social work practice. Works with the health care team to promote positive adjustment, rehabilitation, and improved quality of life for our patients. In collaboration with the interdisciplinary team, informs, educates, and supports staff in understanding the emotional, psychological, and behavioral impact of Chronic Kidney Disease on the patient and family to ensure comprehensive quality care of our patients. Supports the Fresenius Kidney Care (FKC) commitment to the Quality Indicators and Outcomes and Quality Assessment and Improvement (QAI) Activities, including those related to patient satisfaction and quality of life and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FKC Quality Goals. This is an entry-level MSW role.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Patient Assessment / Care Planning / Counseling
- Assess patients' psychosocial status, strengths, and areas of need affecting treatment outcomes as part of the comprehensive assessment.
- Participate in care planning with the patient and healthcare team to identify effective interventions.
- Utilize FKC patient education programs, social work theories, interventions, and quality of life measures to address barriers and meet treatment goals.
- Monitor and intervene to help patients adjust to dialysis and improve psychosocial well-being.
- Provide supportive counseling within scope of clinical training and licensure.
- Offer educational and goal-directed counseling to transplant candidates.
- Assist with referrals to community resources to facilitate treatment outcomes.
- Maintain current knowledge of local vocational/educational programs and assist with patient referrals.
- Assess awareness of advance directives and facilitate discussions with the healthcare team and family.
- Participate in discussions about DNR status to ensure informed decisions.
- Adhere to FMCNA social work policies and documentation standards.
- Document interactions and interventions appropriately.
Quality- Provide psychosocial support addressing adherence, outcomes, and quality of life concerns.
- Participate in monthly quality review meetings and report on relevant indicators.
Patient Education- Assess patient knowledge and barriers related to kidney disease and treatment adherence.
- Provide tailored education about treatment modalities.
- Facilitate transplant referral and manage waitlist processes.
- Educate patients on psychosocial issues and available support services.
- Review patient rights, grievance procedures, and policies.
- Contribute to QAI activities in collaboration with the team.
Patient Admission and Continuity of Care- Review patient rights and address immediate needs upon admission.
- Support scheduling and adjustment processes.
- Identify and communicate root causes for discharge requests to leadership.
Insurance and Financial Assistance- Coordinate with insurance staff regarding coverage issues.
- Provide patient education on payment options and insurance changes.
- Refer patients for billing support as needed.
Staff Related- Assist with hiring processes if requested.
- Maintain updated resource lists and provide staff training on psychosocial topics.
- Participate in team huddles and discuss urgent patient issues.
Adhere to caseload guidelines and perform other duties as assigned.PHYSICAL DEMANDS AND WORKING CONDITIONS:
- Travel may be required for multiple facilities or home visits.
SUPERVISION:
EDUCATION AND CREDENTIALS:
- Masters in Social Work
- State-required license
- Meet licensure requirements in the employment state
EXPERIENCE AND SKILLS:
- 0-2 years of related experience
