PURPOSE AND SCOPE:

Provides psychosocial services to patients treated by the facility, including in-center and home dialysis patients (if applicable), utilizing Social Work Theory of Human Behavior and accepted social work practices. Collaborates with the healthcare team to promote positive adjustment, rehabilitation, and improved quality of life for patients. Supports the Fresenius Kidney Care (FKC) commitment to quality indicators, outcomes, and quality improvement activities, including patient satisfaction and quality of life initiatives. This is an entry-level MSW role.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

1. Patient Assessment / Care Planning / Counseling: Assess psychosocial status, collaborate on care planning, provide counseling, and facilitate referrals to community resources.
2. Quality: Support quality initiatives, participate in reviews, and report on quality indicators.
3. Patient Education: Educate patients about kidney disease, treatment options, and support services.
4. Patient Admission and Continuity of Care: Review patient rights, assist with admission processes, and address immediate concerns.
5. Insurance and Financial Assistance: Liaise with insurance coordinators and educate patients on payment options.
6. Staff Related: Assist in hiring, support staff training, and participate in team meetings.
