Purpose and Scope

Provides psychosocial services to patients treated by the facility, including in-center and home dialysis patients (if applicable), utilizing Social Work Theory of Human Behavior and accepted methods of social work practice. Collaborates with the healthcare team to promote positive adjustment, rehabilitation, and improved quality of life for patients. Supports the Fresenius Kidney Care (FKC) commitment to quality indicators, outcomes, and continuous improvement activities, including those related to patient satisfaction and quality of life. This is an entry-level MSW role.

Principal Duties and Responsibilities
  1. Patient Assessment / Care Planning / Counseling
    • Assess psychosocial status, strengths, and needs affecting treatment outcomes.
    • Participate in care planning with the patient and healthcare team.
    • Utilize education programs, social work theories, and quality of life instruments to address barriers.
    • Provide monitoring and supportive counseling to help patients adjust to dialysis and improve psychosocial well-being.
    • Assist patients seeking transplants with education and referrals.
    • Maintain knowledge of community resources and facilitate access.
    • Assess and facilitate discussions on advance directives and DNR status.
    • Document interactions and interventions appropriately.
  2. Quality
    • Provide psychosocial support addressing adherence and quality of life concerns.
    • Participate in monthly quality review meetings, reporting on relevant indicators.
  3. Patient Education
    • Assess patient knowledge and provide tailored education on kidney disease and treatment options.
    • Support transplant referral processes and educate about support services.
    • Review patient rights, policies, and grievance procedures.
    • Contribute to quality assessment and improvement activities.
  4. Patient Admission and Continuity of Care
    • Review patient rights and address immediate needs.
    • Support scheduling and admission processes to maximize adherence.
    • Identify root causes for discharge requests and communicate concerns to leadership.
  5. Insurance and Financial Assistance
    • Coordinate with insurance staff to address coverage issues.
    • Educate patients on payment options and assist with changes in insurance status.
    • Refer patients to billing solutions for treatment-related questions.
  6. Staff Related
    • Assist with hiring processes if requested.
    • Maintain updated resource lists and provide staff training on psychosocial topics.
    • Participate in team meetings and discuss urgent patient issues.
  7. Adhere to caseload guidelines and perform other duties as assigned.
