Job summary

Ascentria Care Alliance is expanding services in the Keene/Claremont area and seeking a Youth and Family Specialist II with a Master’s level background. You will provide trauma-informed, client-centered therapeutic services to children, families, and adults, with intake, assessment, and ongoing case management across multiple settings.

The role requires bilingual Spanish/English capabilities, clinical supervision experience, and the ability to work across office, home, virtual, and community

Qualifications

• Master's degree in social work, psychology, counseling or related field.
• LSWA certification or eligibility within six months of hire.
• Bilingual in Spanish and English; read, write, and communicate in both languages.

Responsibilities

• Provide direct therapeutic services including individual, group, and family sessions.
• Conduct thorough intake, assessments, and develop treatment plans.
• Perform clinical case management across multiple systems and settings.
• Flexibly provide services in office, foster homes, virtually, and in community.
• Educate and supervise youth in group sessions and activities.
• Coordinate with other providers and build networks and resources.
• Maintain HIPAA compliance and regulatory guidelines.
• Document contacts, progress notes, and complete required documentation.
