Job summary

Windsor Laurelwood Center for Behavioral Medicine in Willoughby, OH seeks a Full-Time Master\'s Level PHP or IOP Counselor to join a multidisciplinary team in IOP and PHP settings. Responsibilities include assessments, treatment planning, group therapy, individual and family counseling, crisis intervention, and discharge planning.

Hybrid remote work may be available based on departmental needs and performance. Requires MSW or Master\'s in Counseling and Ohio license (LSW/LISW/LPC/LPCC).

Qualifications

• MSW or Master's degree in Counseling is required.
• Professional License: LSW/LISW/LPC or LPCC issued by the State of Ohio.
• Two years of experience in mental health or substance abuse treatment with group therapy, crisis intervention, and treatment planning.

Responsibilities

• Conduct psychosocial, mental health, and substance use assessments and determine level of care.
• Develop, implement, monitor, and revise individualized treatment plans.
• Facilitate psychoeducational, process, and skills-based group therapy sessions.
• Provide individual, family, and supportive counseling sessions.
• Conduct crisis intervention, safety planning, and risk assessments as needed.
• Coordinate care with physicians, referral sources, insurance providers, community resources, and family members.
• Develop discharge plans and ensure continuity of care through referrals and communication.
• Complete clinical documentation and compliance requirements.
