Job summary

Windsor Laurelwood Center for Behavioral Medicine in Willoughby, Ohio, seeks a Full-Time Master’s Level PHP or IOP Counselor to join a multidisciplinary team and deliver clinical services in IOP and PHP settings with evidence-based interventions.

The therapist conducts psychosocial and substance use assessments, develops treatment plans, leads groups, provides individual and family therapy, and coordinates care, with hybrid remote work possible based on needs and performance.

Qualifications

• MSW or Master's degree in Counseling from an accredited college or university.
• Must possess Ohio professional license: LSW, LISW, LPC or LPCC.
• Two years' mental health or substance abuse treatment facility experience with group therapy, crisis intervention, and treatment planning.

Responsibilities

• Conduct psychosocial, mental health, and substance use assessments and determine appropriate level of care.
• Develop, implement, monitor, and revise individualized treatment plans.
• Facilitate psychoeducational, process, and skills-based group therapy sessions.
• Provide individual, family, and supportive counseling sessions.
• Conduct crisis intervention, safety planning, and risk assessments as needed.
• Coordinate care with physicians, referral sources, insurance providers, community resources, and family members.
• Develop discharge plans and ensure continuity of care through appropriate referrals and communication.
• Complete clinical documentation, utilization reviews, treatment updates, and compliance requirements.
