Job summary

Windsor Laurelwood Center for Behavioral Medicine in Willoughby, Ohio seeks a Full-Time Master's Level PHP or IOP Counselor to join a multi-disciplinary team. You will perform psychosocial and substance use assessments, develop treatment plans, and facilitate group and individual therapies within IOP/PHP settings.

The role includes crisis intervention, discharge planning, and care coordination with providers and families. Hybrid remote work may be available based on department needs.

Qualifications

• MSW or Master's degree in Counseling from an accredited college or university.
• Must possess one of the following in order to qualify: LSW, LISW, LPC or LPCC issued by the State of Ohio
• Two years previous experience in a mental health or substance abuse treatment facility with direct experience with group therapy, crisis intervention, and treatment planning required.

Responsibilities

• Conduct psychosocial, mental health, and substance use assessments and determine appropriate level of care.
• Develop, implement, monitor, and revise individualized treatment plans.
• Facilitate psychoeducational, process, and skills-based group therapy sessions.
• Provide individual, family, and supportive counseling sessions.
• Conduct crisis intervention, safety planning, and risk assessments as needed.
• Coordinate care with physicians, referral sources, insurance providers, community resources, and family members.
• Develop discharge plans and ensure continuity of care through appropriate referrals and communication.
• Complete clinical documentation, utilization reviews, treatment updates, and compliance requirements.
