Job Description
Summary

Responsibilities
  • Evaluate patients to determine rehab potential, identify needs, develop individualized treatment plans, and set goals within the team plan of care
  • Establish short-term and long-term goals
  • Utilize and maintain equipment safely, including inspection, maintenance, and logs
  • Follow medical regimens and adhere to safety precautions (e.g., THR, TKR, cardiac, wound care)
  • Apply safe treatment techniques such as transfers, positioning, splinting, and modalities like ultrasound and electrical stimulation
  • Complete discharge summaries for each patient at the final visit
Education and Experience
  • Current Physical Therapist licensure in the state of practice
  • CPR certification
  • Valid driver’s license, vehicle insurance, and access to reliable transportation or public transit
