Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking an experienced Marketer / Hospital Liaison to drive census growth and build strong referral partnerships within the healthcare community. The ideal candidate has experience in skilled nursing or related care settings and understands post-acute care pathways.

• Minimum of 2 years of healthcare marketing, business development, admissions, or hospital liaison experience.
• Experience in Skilled Nursing Facility, Rehabilitation Center, Long-Term Care, Home Health, Hospice, or Hospital setting preferred.
• Established relationships with local hospitals and healthcare providers are a plus.
• Strong knowledge of the post-acute care continuum and discharge planning.
• Excellent communication, presentation, and interpersonal skills.

• Collaborate with leadership to develop and execute strategic marketing plans.
• Maintain a thorough understanding of payer sources, managed care plans, and post-acute care services.
• Ensure exceptional customer service throughout the referral and admission process.

The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking an experienced, energetic, and relationship-driven Marketer / Hospital Liaison to support census growth and strengthen referral partnerships throughout the healthcare community.

The ideal candidate will have experience in skilled nursing, rehabilitation, long-term care, or healthcare marketing and possess a strong understanding of the post-acute care continuum.

Position Summary

The Marketer / Hospital Liaison serves as the primary point of contact for hospitals, case managers, physicians, discharge planners, and community partners to promote the facility's clinical programs and services. This role is responsible for developing referral relationships, increasing admissions, and ensuring a seamless transition for patients entering our center.
