Job summary

FeldCare Connects is seeking a self-motivated Physical Therapist to deliver premier care to adults in the Los Angeles area. The PT in Home Health will assess patient needs, develop and execute a treatment plan to reduce pain, restore function, and promote independence.

Licensure in good standing is required; bilingual skills are a plus. The role emphasizes comprehensive assessment, plan development, and ongoing progress monitoring in a home health setting, with strong communication to patients

Qualifications

• Active Physical Therapist license and state registration.
• Graduation from a PT curriculum approved by The American Physical Therapy Association, The Council on Medical Education and Hospitals of the American Medical Association, or The Council on Medical Education of the AMA.
• Bilingual a plus.

Responsibilities

• Assess and evaluate patient care needs to promote movement, reduce pain, restore function and prevent disability.
• Determine and implement a treatment plan to improve and enhance patient well-being.
• Execute interventions to improve mobility and monitor patient progress.
• Communicate effectively with patients, families, and the care team; document care clearly and accurately.
