Job summary

CAN Community Health, Inc. is seeking a Manager, Prevention to lead education and prevention initiatives focused on HIV, hepatitis C, STIs, and related conditions. The role demands grant development, reporting, data monitoring, and strong leadership within a mobile and office-based program.

The position emphasizes compliance with HIPAA as well as quality assurance and continuous improvement. A Bachelor’s degree is preferred with at least four years of relevant experience.

Qualifications

  • Bachelor’s degree preferred but not required.
  • Minimum 4 years’ experience in related field.
  • Personnel and program management experience.
  • Experience with development and implementation of HIV Prevention procedures.
  • Must be able to operate a motor vehicle and have valid insurance and driver’s license.

Responsibilities

  • Development and implementation of grants and new CAN initiatives.
  • Completes and submits necessary reports to state/local funding sources with accurate data.
  • Monitors program and grant progress monthly and aligns team operations.
  • Resolves problems and ensures compliance with regulations and standards.
  • Ensures quality management of prevention programs and participates in QA/QI efforts.
  • Assists in grant writing and/or grant summary reporting as necessary.
  • Assist in internal and external program audits.
  • Analyzes epidemiological data to guide outreach and day-to-day operations.
  • Accurately documents and maintains prevention data records.
  • Ensures client confidentiality per HIPAA and CAN policies.

CAN Community Health is now hiring a Manager, Prevention

Standard clinic hours are Monday-Thursday 8:00 am-5:30pm Friday 8:00 am-12:30 pm. This position requires hours of work outside normal operating hours including nights and weekends.

What You’ll Do

The Program Manager is an advanced position within CAN Community Health’s Education & Prevention Team. This role requires a high level of knowledge and skill in community development, grant writing and management, grassroots program development and sustainability, operations implementation in office and mobile units for HIV prevention and care programs. As well as personnel management and leadership.



  • Development and implementation of grants and new CAN initiatives.

  • Completes and submits necessary reports as required by state and local funding sources. Ensures accurate data collection and reporting in line with grant obligations.

  • Monitors program and grant progress on a monthly basis and align team operations to ensure deliverables are met.

  • Resolves problems and ensures compliance with regulations and standards.

  • Ensures quality management of all prevention programs. Participate in QA/QI efforts.

  • Assists in grant writing and/or grant summary reporting as necessary.

  • Assist in internal and external program audits.

  • Analyzes and utilizes federal, local and state epidemiological data to formalize outreach programs and guide day-to-day operations.

  • Accurately documents and maintains records as related to prevention data collection efforts.

  • Ensures client confidentiality 100% of the time by conforming to HIPAA laws and CAN Community Health Policies and Procedures.

Education/Professional:
