Description
The Manager of Clinical Services is responsible for ensuring that patient care is coordinated and managed appropriately. This position oversees the delivery of care and supervises clinical personnel.
Essential Job Functions/Responsibilities
- Receive case referrals, review patient information, including home visits, and determine hospice/home care needs. Assign appropriate personnel and consult with Medical Director or Hospice Physician regarding patient eligibility.
- Evaluate each case through home visits, record reviews, and clinician services; guide clinicians to improve performance and care quality.
- Be available during operating hours to assist clinicians as needed.
- Review patient's medical diagnosis, prognosis, medications, procedures, and clinical course.
- Assist clinicians in establishing therapeutic goals, setting priorities, and developing care plans.
- Participate in case conferences to coordinate care and discuss interdisciplinary involvement.
- Conduct quarterly record reviews, communicate findings to leadership and staff.
- Assist in hiring, onboarding, and evaluating staff, including annual reviews.
- Maintain patient clinical records, statistics, and reports in compliance with laws.
- Ensure adequate inventory of supplies and equipment.
- Support staff education and training programs, and implement organizational policies and goals.
- Follow professional standards and principles.
- Assess community health needs and participate in public relations and community activities.
- Promote customer service among hospice personnel.
Additional Duties
- Participate in on-call duties and interdisciplinary meetings.
- Engage in professional development activities.
- Contribute to quality improvement teams and initiatives.
Qualifications
- Current RN license in applicable state(s).
- Graduate of an accredited nursing program; associate degree required, bachelor’s preferred.
- At least 2 years of management or supervisory experience in hospice/home care preferred.
- Ability to work within an interdisciplinary team.
- Ability to pass DHS background check.
- Valid driver’s license, insured vehicle, and acceptable driving record if driving.
The salary range is $95,000-$115,000 annually, with benefits including insurance and 401(k). Compensation varies based on experience, location, and organizational needs.
