Job Summary

Responsible for directing daily operations of Case Management nursing staff to ensure effective workflow and patient care. Establishes goals, assesses organizational structure, and implements process improvements to optimize clinical and cost outcomes. Serves as primary communication liaison with clinical leadership and promotes Case Management initiatives. Manages staffing needs through recruitment, training, coaching, and corrective action. Ensures compliance with payer guidelines, coordinates interdisciplinary care, and escalates complex cases for resolution.

Essential Functions
  • Aligns and sustains resources and standard work to ensure consistent, standardized social services and optimal patient outcomes.
  • Oversees daily operations, policy development, and implementation for social work and case management services.
  • Hires, supervises, and provides disciplinary action for staff, fostering high employee engagement and professional growth.
  • Utilizes data, metrics, and performance indicators to drive departmental decisions, process improvements, and positive patient transitions.
  • Ensures compliance with government, regulatory, and payer requirements, including Medicare, Medicaid, and third-party guidelines.
  • Manages departmental budgets and achieves strategic and financial goals.
  • Collaborates with medical, interdisciplinary, and quality teams to coordinate care, facilitate communication, and support enterprise objectives.
  • Maintains and reports key metrics, dashboards, and outcomes to leadership and stakeholders.
  • Performs other duties as assigned.
Education Requirement

Bachelor’s degree in social work, nursing, psychology, sociology, or a related field.

Experience Requirement

4-5 years of experience in nursing, case management, or a relevant area. An equivalent combination of education and experience may be considered. All experience must be paid and in the same related field. Part-time and PRN experience will be prorated based on hours worked per week. Volunteer work and internships for academic credit are not counted.

Certifications & Licensures

Current and active Registered Nurse (RN) license, Certified Case Manager (CCM) or Certified Professional in Healthcare Quality (CPHQ)
