Qualifications

Collaborates with multidisciplinary teams to develop and implement comprehensive care and discharge plans. Communicates effectively with patients, families, and healthcare partners to ensure continuity, satisfaction, and optimal outcomes. Utilizes clinical assessments and evidence-based practices to guide resource utilization and prevent readmissions. Supports quality improvement initiatives and serves as a resource for education and training in case management. ## Essential Functions• Conducts comprehensive assessments of clinical discharge needs for patients.• Coordinates multidisciplinary plans of care addressing discharge and anticipatory clinical needs.• Facilitates care coordination, case finding, screening, eligibility determination, and ongoing monitoring and evaluation of care plans.• Rounds with clinical staff to provide discharge planning recommendations and reviews care plans for goal consistency and prevention of healthcare-acquired events.• Organizes and expedites access to tests, procedures, and results to prepare patients for discharge and reduce length of stay.• Arranges post-discharge services, including home health, durable medical equipment, and transfers to appropriate agencies or facilities.• Serves as a liaison with internal and external departments and agencies to ensure continuity of care and transitions.• Maintains up-to-date clinical knowledge of available services and eligibility criteria, and educates patients, families, and staff accordingly.• Documents clinical needs, services, and discharge plans in the medical record and communicates with all relevant stakeholders.• Performs other duties as assigned. ## Education RequirementBachelor’s degree in social work, nursing, psychology, sociology, or a related fieldMaster’s degree in nursing is preferred ## Experience Requirement2-3 years of experience in nursing, or a relevant area An equivalent combination of education and experience may be considered.All experience must be paid and in the same related field.Part-time and PRN experience will be prorated based on hours worked per week.Volunteer work and internships for academic credit are not counted. ## Certifications & LicensuresActive Registered Nurse license ## Working ConditionsA.
