Job summary

Catholic Health seeks a Manager of Care Coordination (MCC) – Registered Nurse (RN) to coordinate multidisciplinary care from admission to discharge in a patient-centered setting.

The role emphasizes leading teams, removing barriers to discharge, and acting as a clinical resource for staff, ensuring safe, high-quality progression and optimal bed management.

Qualifications

• 1+ years of Case Management experience as an RN.
• 1+ years of nurse leadership experience.
• Education: BSN; Licensure: NY RN License & Registration; BLS certification.

Qualifications

• Passion for Care Coordination
• 1+ years of Case Management experience as a Registered Nurse (RN)
• 1+ years of nurse leadership experience
• Education: Bachelors of Science in Nursing (BSN) degree
• Licensure: New York Registered Nurse (RN) License & Registration
• Certification: Basic Life Support (BLS)

Salary Range

USD $65.20 – $78.24 per hour.
