Job Summary And Responsibilities

Nurse Manager of Care Coordination / Case Management at Mercy San Juan Medical Center, reporting to the Director of Care Coordination and overseeing a department of 35 FTEs (RNs and CCA). Responsible for daily operations oversight to ensure efficient, compliant execution of Care Coordination goals and key performance metrics. Duties include staff performance management, data review for performance improvement, participation in meetings and projects related to departmental functions, communicating organizational goals to staff, and advocating for team needs to leadership.

Principal Duties And Accountabilities

Under the guidance of the Department Director, responsible for oversight of daily operations to ensure efficient and compliant execution of the case management functions. Duties include, but are not limited to:

• Managing areas including organizing and executing all activities, staffing, performance improvement in the delivery of clinical services (such as LOS reduction), and reporting needs within the Medical Center and Dignity Health, as well as government and regulatory reporting.
• Implementing Care Coordination activities according to the needs, requirements, and policies of the Medical Center, the affiliated medical groups and health plans, Dignity Health, any federal and state agencies, and according to standard practices of the professions under the manager’s accountability.
• Overseeing the department in an efficient manner to ensure timely and compliant care coordination, discharge planning, utilization review and social work interventions resulting in quality patient care.
• Consulting and collaborating with other managers, physicians, administration, and community-based healthcare workers regarding care management issues identified through corporate or facility initiatives and current literature.

Job Requirements

• Minimum of three (3) years experience in Care Coordination or Case Management.
• Bachelor’s degree in Nursing or equivalent experience.
• Current California RN licensure.

Preferred

• Leadership experience.
• Care Coordination certification.

Pay Range

$60.24 – $89.60 per hour
