Job summary

Your Health is seeking a Nurse Practitioner to join our growing primary care team serving Macon, GA and surrounding areas. You will perform patient visits in homes and other settings, coordinating with a multidisciplinary team to ensure comprehensive, high-quality care.

As a full-time provider, you will manage high-risk patients, develop individualized care plans, educate patients and families, and participate in care coordination efforts.

Qualifications

• NP/PA/Physician with valid state license in good standing.
• Experience in home health, clinics, facilities, telehealth, or specialty care settings.
• Strong communication and teamwork skills.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity of care and optimal outcomes.
• Communicate regularly with case managers, coordinators, pharmacists, health coaches and other team members to coordinate patient care.
• Participate in interdisciplinary team meetings to discuss patient progress and care plans.
• Document care management activities accurately and efficiently to support patient care plans and regulatory compliance.
• Provide patients with education and resources related to their conditions and self-management.
• Travel between offices, facilities, and patient homes, and adapt to flexible hours.
