Your Health Organization in Macon, GA seeks a qualified Nurse Practitioner to provide in-home patient care as part of a dedicated care team. You will evaluate high-risk patients, coordinate plans with clinicians, and deliver comprehensive care management across homes, clinics, and telehealth settings.

The role requires collaboration, strong communication, and the ability to travel between offices and locales. A full-time salary-based position includes a monthly bonus opportunity.

Qualifications

• Nurse Practitioner, PA, or Physician licensed in the state of employment.
• Experience in home health, clinic, facility, telehealth, or specialty care setting.
• Strong communication, collaboration, and interpersonal skills in an interdisciplinary team.
• Ability to evaluate high-risk patients, develop care plans, and coordinate care.
• Commitment to compassionate, patient-centered care and excellence in healthcare delivery.

Responsabilités

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity of care.
• Communicate regularly with case managers, coordinators, social workers, and specialists to coordinate patient care.
• Participate in team meetings to discuss patient progress and care plans.
• Document care management activities accurately to support patient plans and regulatory compliance.
• Provide patients with education and resources related to their conditions and preventive care.
