Sequoia Living, Inc. in Walnut Creek, CA is seeking an LVN Wellness Supervisor to oversee Certified Nursing Assistants across Independent, Assisted Living, and Memory Care.

You will ensure safe, compassionate care, support goal-directed service plans, and coordinate with the care team to maintain quality and dignity for every resident. The role requires licensure as an LVN in California and 2–5 years of nursing experience including supervision.

Qualifications

• Graduation from an accredited school of nursing and LVN licensure in California.
• Minimum 2–5 years nursing experience including direct supervisory responsibility.
• Experience in geriatric settings preferred.
• Ability to supervise up to 10 CNAs and coordinate with care teams.

Responsibilities

• Supervises CNAs ensuring daily resident care in accordance with policies.
• Participates in hiring, orientation, training, and performance management of staff.
• Contributes to interdisciplinary meetings and resident care planning.
• Ensures nursing policies are implemented and safety procedures followed.
• Documents health records electronically and maintains accuracy.
• Schedules CNAs to ensure adequate coverage and supports admissions.
• Provides personal assistance with daily living where needed and updates service plans.
• Communicates residents' conditions with physicians, families and staff.
• Reviews nursing notes and orders for regulatory compliance.
• Maintains resident roster, census, and staffing coverage as needed.

Job description

We are hiring a LVN Wellness Supervisor who is responsible and accountable for supervising the Certified Nursing Assistant (CNA) staff, ensuring quality care, maximum independence, and self-direction for Independent, Assisted Living, and Memory Care residents. Provides direct and indirect resident care services that ensure the safety, comfort, personal hygiene, and protection of residents; includes the performance of disease prevention and restorative measures. What You Will Do:

• Supervises and ensures Certified Nursing Assistants carry out their daily responsibilities and care of residents in accordance with Sequoia Living and Viamonte policies and procedures.
• Participates in hiring, orientation, training & coaching, performance management and supervision of facility certified staff.
• Participates in or conducts, in Director of Wellness’s absence, interdisciplinary team/ resident care and other appropriate meetings. Shares on-call responsibilities with the Director of Wellness. Fills in for Director of Wellness in his/her absence.
• Ensures nursing policies and procedures are implemented.
• Ensures staff uses proper lifting and resident safety techniques when lifting, transferring, and ambulating patients.
• Documents accurate health records electronically.
• Oversees scheduling of CNAs to ensure adequate coverage, in coordination with the Medical Administrative Specialist.
• May facilitate orientation and admission of new residents and family members.
• Provides personal assistance with daily living and personal care in resident’s independent living, assisted living, and memory care apartments in accordance with an individual's goals and/or service plan. Updates and maintains resident service plans.
• Communicates with outside physicians, residents, families, administrative personnel, and appropriate staff regarding residents’ current physical and mental condition and resident care needs.
• Reviews nursing notes, physician orders, and progress notes for compliance with state and federal regulations.
• Regularly maintains and updates resident roster and census.
• Provides coverage as needed based on staffing needs.
• Performs related work as required.

Who You Are:

• Interpersonal skills to establish and maintain good working relationships among unit staff, other department and medical staff. Basic computer skills in a Windows environment. Sensitivity and understanding of issues related to aging. Patience and tact in dealing with residents, staff, and visitors. Knowledge and skill to make decisions regarding unit activities. Leadership skills to supervise and motivate staff. Knowledge of facility philosophy and policies. Thorough knowledge of current nursing theory and practice. Knowledge of Title 22 and OBRA regulations. Computer skills to document into Electronic Health Records system (MatrixCare). Physical skills and ability to perform work that requires continual standing, walking, stooping, bending and lifting up to 50 pounds.
• Supervises up to 10 CNAs. Responsible for the selection, training, motivation, performance appraisal and discipline of assigned employees in conjunction with the Director of Wellness. May act as Director of Wellness in his/her absence.
• Graduation from an accredited school of nursing and licensure to practice as a LVN in the State of California. Experience equivalent to minimum 2-5 years nursing experience, including direct supervisory responsibility, preferably in a geriatric setting
