Evergreen Healthcare Group in Daly City is seeking a Treatment Nurse to provide specialized nursing care focused on wound care, skin integrity, and physician-ordered treatments for residents in a Skilled Nursing Facility. The role emphasizes accurate documentation, infection control, and collaboration with the care team.

You will document care, monitor wounds for infection, and participate in rounds with physicians and interdisciplinary colleagues.

Qualifications

• Current and active LVN license in California.
• Experience in long-term care or skilled nursing preferred.
• Experience with wound care, skin assessments, and treatment documentation preferred.
• Knowledge of infection control practices and regulatory standards.
• Strong assessment, documentation, and organizational skills.
• Ability to work collaboratively with an interdisciplinary care team.

Responsibilities

• Perform and document physician-ordered treatments, including wound care, dressing changes, and skin treatments.
• Assess, monitor, and document wounds, pressure injuries, and other skin conditions.
• Collaborate with physicians and the interdisciplinary care team to develop and implement treatment plans.
• Maintain accurate and timely documentation in the medical record.
• Monitor residents for signs of infection or complications and report changes in condition.
• Ensure compliance with infection control protocols and wound care best practices.
• Participate in weekly wound rounds and interdisciplinary care meetings.
• Educate nursing staff on proper wound care procedures and prevention of skin breakdown.
• Assist with implementing interventions to prevent pressure injuries.
• Maintain treatment supplies and ensure proper storage and infection control standards.
• Ensure treatments are completed according to physician orders and facility schedules.

Job description

The Treatment Nurse is responsible for providing specialized nursing care focused on wound care, skin integrity, and physician-ordered treatments for residents in a Skilled Nursing Facility. This role ensures treatments are completed accurately, documented appropriately, and in accordance with physician orders, facility policies, and regulatory guidelines.

The Treatment Nurse plays a key role in promoting healing, preventing infection, and maintaining the highest level of resident care and comfort.

Responsibilities

• Perform and document physician-ordered treatments, including wound care, dressing changes, and skin treatments
• Assess, monitor, and document wounds, pressure injuries, and other skin conditions
• Collaborate with physicians and the interdisciplinary care team to develop and implement treatment plans

• Ability to stand, walk, bend, and lift up to 50 pounds
