Job summary

Silvergate Rancho Bernardo is seeking an LVN Supervisor to join its premier retirement community in Rancho Bernardo. You will support the Director of Resident Care in supervising caregivers and MedTechs across Independent, Assisted Living, and Dementia care.

Responsibilities include staffing, assessments, medication oversight, and coordination with physicians, families, and home health providers. Must be CA-licensed LVN with CPR; 50 lb lift capability may be required.

Qualifications

• Must be a licensed LVN in California with current CPR certification.
• Experience supervising caregivers and MedTechs preferred.
• Strong documentation and communication skills required.

Responsibilities

• Oversee supervision and training of caregivers and MedTechs.
• Assess residents, document changes, and notify physicians/families as needed.
• Manage medication administration and patient care plans.
• Coordinate medical appointments and liaise with home health agencies.
• Ensure compliance with Title 22 and confidentiality standards.

Silvergate Rancho Bernardo is the third Silvergate community in San Diego county, joining its sister communities in Fallbrook and San MarcosJOB SUMMARYProvides support to the Director of Resident Care and Assistant Director of Resident Care in overseeing the supervision and training of Caregivers and MedTechs in the provision of personal services to residents in Independent, Assisted Living and Dementia/Alzheimer’s disease care.ESSENTIAL FUNCTIONS AND RESPONSIBILITIESEvaluates staffing needs on a continuous basis to ensure that positions are filled in a timely manner to minimize department overtime and provide continuous quality service to residentsAssesses residents to determine medical problems and/or changes in condition and ensures proper documentation in charts; also notifies physicians and family members of condition changes or medical problemsOversees the monitoring and charting of daily care of residentsReviews physician’s orders, lab requests and follows up with residents as neededPerforms finger sticks and insulin injections for diabetic residentsMonitors any special dietary requirements of residents and ensures caregivers are following orders received from DirectorMaintains supplies, including stocking and ordering new suppliesResponds to resident and family telephone inquiriesAssist with the coordination of medical appointments for residentsProactive in obtaining medical care and treatment for residents with medical needs, working to minimize emergency 911 calls as much as possibleResponsible for ensuring caregivers have clear direction for the direct care that is delivered to the residentsWork with various Home Health/Hospice agencies to ensure residents on Hospice care are kept comfortable and the terms of their care plan are carried outExercise proper use of equipment and instructs others to do the sameMaintain a high level of confidentialityPromote a positive work environment that emphasizes teamworkPerform other duties as assignedQUALIFICATIONSMust be at least 18 years of ageGraduate of an accredited nursing program, with LVN licenseCurrent and valid State of California licensure for Licensed Vocation Nurse and current CPR certificationMust have First Aid CertificationKnowledge of State of California requirements set forth in Title 22Knowledge and understanding of Gerontology, Dementia and Alzheimer’s diseaseEnjoy working with senior citizensAbility to lead, direct and work well with othersPrevious experience working closely with lead managementExcellent time management and organizational skillsKnowledge and ability to oversee medication programProficiency in Microsoft Office programs, including ability to send emails, generate letters and other correspondence, work with spreadsheets; Also able to use other various office machinery, including a fax machine and copy machineKnowledge of resident rights and the ability to respond to each resident with empathy and respectAbility to communicate in English clearly and effectively, both orally and in writingAbility to establish and maintain effective working relationships with coworkers, other departments, resident and their family members, doctors and other outside professionals that provide service to the residentsAbility to respond effectively in an emergency or crisis situationPHYSICAL REQUIREMENTSThis position requires the ability to bend, squat, stoop, climb, kneel, and stand on feet for extended periods of time, as well as the ability to lift and carry up to 50 pounds.SPECIAL REQUIREMENTSJob offer is contingent on the following:Must pass background checkMust pass drug test, pre-employment physical and TB testProof of COVID vaccine required
