Job summary

The Legacy Senior Communities in Dallas, TX is seeking a registered nurse responsible for implementing and evaluating nursing care prescribed for residents. The role requires adherence to the nurse practice act, state and federal regulations, and ensuring high-quality patient care.

You will perform admissions and ongoing assessments, maintain care plans, document medications, supervise nursing staff, and protect residents' privacy and Advance Directives while supporting nutritional needs.

Qualifications

  • Graduate from an accredited School of Nursing.
  • Proficient in nursing assessments, care planning, and documentation.
  • Knowledge of state/federal nursing center regulations.
  • Ability to administer and document medications accurately.
  • Provide and monitor medical treatments as ordered.

Responsibilities

  • Implement and evaluate nursing care prescribed for residents.
  • Perform admissions and periodic assessments of residents.
  • Review and revise care plans and physician orders.
  • Administer medications following the five rights (right medication, route, resident, time, dose).
  • Monitor residents and report changes to physicians and proxies.
  • Respect residents' Advance Directives and privacy.
  • Assist with meeting nutritional/hydration needs and document accordingly.
  • Supervise nursing aides and medication aides during shifts.
  • Maintain confidentiality of residents' health information.

Job description

Responsible for the implementation and evaluation of all nursing care prescribed and provided to the resident.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

  • Provides services in accordance with nurse practice act and state/federal nursing center regulations.
  • Performs admission and periodic assessments on residents for functional and mental abilities.
  • Reviews/revises assessments and care plans at admission, periodically per regulation, and at change in resident physical or psychosocial condition.
  • Reviews and transcribes physician's orders.
  • Administers and documents medications in accordance to the five rights: right medication, right route, right resident, right time, and right dose.
  • Provides medical treatments as ordered by physician.
  • Supervises and monitors medications for residents who self-administer.
  • Observes residents and reports condition changes to physicians, health care proxies, and supervisors.
  • Understands the resident’s Advance Directives and follows his/her wishes.
  • Assists resident with meeting their nutritional/hydration needs and documents on the Nursing record appropriately.
  • Monitors meal-time and nourishments, assists with trays, and helps feed residents.
  • Maintains confidentiality of residents’ private health information.
SUPERVISORY RESPONSIBILITIES
  • Assist with assignment making for team members and follow-up to ensure high standards of care and documentation of such care.
  • Helps to supervise nursing aides and medication aides during shift.
