Job summary

Assisted Home Health & Hospice is seeking LVNs for shift work in the San Fernando Valley. New grads welcome; training provided. You will deliver in-home nursing care to one patient at a time, in a stress-free environment. Pay is $28/hr with benefits if you average 30 hours per week.

This role requires graduation from a LVN program and California LVN licensure. Work setting involves coordination with RN Case Manager and physician to ensure quality care and adherence to the plan of care.

Qualifications

• Graduate of accredited practical (vocational) nursing program.
• Licensed LVN by the California Board of Vocational Nurse and Psychiatric Technician Examiners.

Responsibilities

• Provide quality home health nursing care to one patient at a time.
• Coordinate with RN Case Manager and physician regarding patient changes and needs.
• Assist RN and/or physician with specialized procedures and carrying out the plan of care.
