Job summary

Healogics in Oklahoma City is hiring a Wound Care Center Licensed Practical Nurse (LPN). Under the direction of an RN or provider, you will deliver wound care and related nursing services in a fast-paced outpatient setting.

This role requires a practical nursing credential, BLS, and at least two years of LPN/LVN experience (SNF/LTACH experience preferred). You will collaborate with a multidisciplinary team, maintain accurate documentation, and support patient intake, discharge, and case

Qualifications

• High School Diploma or GED required.
• Graduate from an accredited practical nursing program.
• Current LPN/LVN license in the state of practice.
• Two or more years of LPN/LVN experience, with wound care preferred.

Responsibilities

• Under RN/Provider direction, provides wound care per BLS and hospital protocols.
• Assist in vein clinic after Vein Clinic training.
• Assist with intake and discharge.
• May support case management under RN supervision.
• Gathers patient information and necessary equipment.
• Reports quality issues to the Clinical Nurse Manager.
• Communicates with team regarding interventions for quality care.
• May function as Documentation Assistant according to policy.
• Performs other duties as delegated.
