Job summary

Healogics is seeking a motivated LPN/LVN to join our Wound Care Center, working under the direction of an RN or provider to deliver wound-related nursing care in a hospital setting. The role includes assisting with intake/discharge, contributing to case management, and coordinating with the care team to ensure patient safety and quality outcomes.

The ideal candidate has an active LPN/LVN license, 2+ years of nursing experience (preferably in SNF/LTACH), and strong communication skills.

Qualifications

• LPN/LVN license in the state of practice and maintain throughout employment.
• 2+ years of nursing experience, prefer SNF/LTACH settings.
• Wound care experience preferred.
• BLS certification required.
• Strong communication and teamwork abilities.

Responsibilities

• Provide wound-related patient care under RN/Provider guidance.
• Assist in vein clinic tasks after Vein Clinic training.
• Support intake and discharge processes.
• Assist with case management under RN supervision where allowed by state law.
• Collect patient information and prepare equipment.
• Report care quality issues to Clinical Nurse Manager.
• Communicate with team to coordinate therapeutic interventions.
• Perform hyperbaric therapy-related duties as trained; follow safety protocols.
