Job summary

Sweetwater Care is seeking an experienced LVN to provide nursing care in a skilled nursing facility environment. The incumbent will manage patient care within scope of practice and follow physician orders, facility policies, and applicable guidelines.

Responsibilities include infection control adherence, quality improvement participation, patient education, and coordinating with physicians. The role requires timely medication administration and collaboration with departments on patient changes

Qualifications

  • Must hold a current LVN license in the state.
  • Minimum of 1 year of nursing management experience preferred.
  • Experience in skilled nursing facility (SNF) / long-term care (LTC) preferred.

Responsibilities

  • Demonstrates knowledge and appropriate use of the Infection Control Manual.
  • Participates in implementing the Quality Improvement Program.
  • Uses the Quality Life Manual for patient placement and monitoring.
  • Notifies other departments of patient changes (room changes, LOA, admissions, discharge).
  • Conducts family and patient education.
  • Administers medications timely and per facility policy.
  • Administers treatments timely and per facility policy.
  • Receives, transcribes, and carries out physician orders, when allowed by Nurse Practice Act.
  • Makes rounds with physicians as needed.
