Sundance Creek Post Acute is seeking a compassionate LVN for a part-time role. You will deliver direct nursing care, support resident well-being, and collaborate with physicians, nurses, therapists, and other team members to maintain high clinical standards.

Responsibilities include administering medications, monitoring residents, documenting care, and contributing to a supportive, resident-centered environment. California LVN license required.

Qualifications

• Excellent communication and interpersonal skills.
• Strong organizational and clinical assessment abilities.
• Ability to work effectively in a fast-paced healthcare environment.
• Previous SNF/post-acute/long-term care experience preferred but not required.

Responsabilités

• Provide skilled nursing care per physician orders and care plans.
• Administer medications and treatments accurately and safely.
• Monitor resident conditions and report changes promptly.
• Communicate effectively with residents, families and interdisciplinary teams.
• Maintain accurate clinical documentation.
• Support infection control, safety and regulatory standards.
• Contribute to a positive, team-focused culture centered on resident care.
