Kern River Transitional Care Center in Bakersfield, CA is seeking a compassionate Licensed Vocational Nurse (LVN) for the Full-Time NOC Shift to join our nursing team. We offer a collaborative environment focused on high-quality resident care.

As an LVN, you will provide direct care, administer medications, monitor residents, document thoroughly, and work with an interdisciplinary team to ensure exceptional care. Competitive pay with a $1.00/hr NOC differential and strong benefits are available.

Formación

• Current California LVN license in good standing.
• Current CPR/BLS certification as required.
• Experience in skilled nursing, long-term care, rehabilitation, or post-acute care preferred.
• Strong clinical assessment and documentation skills.
• Excellent communication and organizational abilities.

Responsabilidades

• Provide direct nursing care to residents per physician orders and care plans.
• Administer medications and treatments accurately and timely.
• Monitor residents for changes in condition and report concerns promptly.
• Complete accurate nursing documentation.
• Participate in development and implementation of individualized care plans.
• Communicate effectively with residents, families, physicians, and nursing staff.
• Assist with admissions, transfers, and discharges as needed.
• Supervise CNAs as assigned.

Licensed Vocational Nurse (LVN) - Full-Time NOC Shift

Kern River Transitional Care Center is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our nursing team on the Full-Time NOC Shift. We are looking for a dedicated nurse who is committed to providing excellent resident care and working collaboratively with our interdisciplinary team.

Compensation

Pay Range: $28.00-$32.00 per hour, based on experience

NOC Shift Differential: Additional $1.00 per hour

Essential Duties and Responsibilities

• Provide direct nursing care to residents in accordance with physician orders, resident care plans, and facility policies.
• Administer medications and treatments accurately and timely.
• Monitor residents for changes in condition and promptly report concerns to the appropriate physician or nursing supervisor.
• Complete accurate and timely nursing documentation.
• Participate in the development and implementation of individualized resident care plans.
• Communicate effectively with residents, families, physicians, nursing staff, and members of the interdisciplinary team.
• Assist with admissions, transfers, and discharges as needed.
• Obtain and document vital signs and other clinical information.
• Ensure physician orders are carried out accurately.
• Supervise and provide direction to CNAs as assigned.
• Follow infection prevention and control procedures.
• Respond appropriately to resident emergencies and changes in condition.
• Maintain confidentiality of all resident and facility information.
• Ensure residents are treated with dignity, respect, and compassion.
• Follow all federal, state, and facility nursing regulations, policies, and procedures.
• Perform additional nursing duties as assigned.

Qualifications

• Current and active California LVN license in good standing.
• Current CPR/BLS certification as required.
• Previous skilled nursing, long-term care, rehabilitation, or post-acute experience preferred.
• Strong clinical assessment and documentation skills.
• Excellent communication and organizational abilities.
• Ability to prioritize resident needs and manage multiple responsibilities.
• Ability to work effectively with residents, families, nursing staff, and other healthcare professionals.
