Job summary

Crossover Health is seeking a licensed LVN/LPN to join our clinic team, delivering high-quality patient care under RN supervision. You will assist with check-ins, coordinate with specialists, and help patients navigate benefits within our primary health model.

Responsibilities include taking histories, vitals, phlebotomy, and immunizations while maintaining a patient-centered, supportive environment in a fast-paced clinic. On-site role with a focus on value-based care.

Qualifications

• Licensed Vocational Nurse with a minimum of 3 years of clinical experience.
• Current BLS (Basic Life Support) certification.
• Ability to thrive in a fast-paced, changing clinic environment.

Responsibilities

• Delivers patient care under the oversight of an RN or clinic supervisor.
• Coordinates care with specialists and facilities to support patient flow.
• Gathers medical histories and vitals to aid clinician assessment.
• Provides phlebotomy and immunization services when needed.
• Supports member check-in and insurance benefit understanding.
• Engages with members to answer questions and schedule appointments.
