Job summary

Crossover Health is seeking a Licensed Vocational Nurse (LVN/LPN) for our Mountain View clinic to support a patient-centered medical home. You will coordinate care with an interdisciplinary team, perform essential nursing duties, take histories, monitor vitals, and assist clinicians in a fast-paced environment.

The role requires a CA LVN license, 3+ years of clinical experience, and a current BLS certification.

Qualifications

• Active LVN/LPN license in California
• Minimum of 3 years clinical experience
• Current BLS certification required

Responsibilities

• Delivers patient care under RN or clinic supervisor
• Collaborates with care team to support a patient-centered medical home
• Provides evidence-based nursing care per community standards
• Takes medical histories, records vitals, and assists clinicians
• Coordinates referrals with specialists and facilities
• Provides phlebotomy and immunization services
• Performs other duties as assigned
