AccentCare, Inc. in Madison, MS is seeking an LVN/LPN for direct patient care under RN supervision. This full-time role covers pediatric to geriatric patients and involves documenting progress and nursing status changes.

Work schedule Mon-Fri 8:00 am-4:30 pm with one weekend per month on call. Salary is estimated at $48,000–$54,000 per year, paid per point. Benefits include medical/dental/vision, PTO, 401(k) match, and more.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation.
• One year of LVN/LPN work experience.
• Must possess and maintain valid CPR certification while employed in a clinical role.
• Must be a licensed driver who can travel to all business locations.

Responsibilities

• Performs technical nursing functions and direct patient care under the supervision of an RN.
• Promotes and maintains optimal client care under the supervision of an RN.
• Maintains a safe patient environment.
• Identifies and communicates significant abnormalities in patients data to case manager and other home health team members.
• Administers prescribed medications and treatments as permitted by law.
• Re-evaluates client’s nursing needs and reports findings to Case Manager.
• Counsels and instructs the client and family in meeting nursing and related needs.
• Prepares clinical and progress notes.
• Submits all documentation in a timely manner in accordance with agency policy.
• Documentation on patients chart correlates with nursing care plan.
• Communicates frequently and works closely with other home care team members.
• Assists in coordinating services provided to client
